Wondering if you should schedule a visit with a Behavioral Health provider?

Wondering if you should schedule a visit with a Behavioral Health provider? Taking one (or both) of these simple questionnaires may help you decide. Questionnaires are NOT intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the guidance of your doctor or other qualified health professional with any questions you may have regarding your health or a medical condition.

  • 1. For each question, circle the number that best matches your mood or thinking for the past two weeks.
  • 2. Add up each column and write the total on the corresponding line underneath question 9.
  • 3. Add together the total for each column for your final score.
  • 4. A score of 9 or greater may indicate a need to see a Behavioral Health provider.
