How do I make an Appointment?

Request a visit (in-person, video or “curbside”) through MyChart or call:

What is a Video Visit?

A Video Visit lets you see your provider from home through a computer or smart device.

What is a Curbside Visit?

A “Curbside” visit is a Video Visit from your vehicle, at a clinic parking lot, using an iPad.

How do I make a same-day appointment?

Call your clinic to request a sick visit and a nurse will help get you scheduled.

Can I walk in?

While we can accommodate walk-in services, for your safety and convenience, please call ahead if possible.

Do you accept new patients?

Yes! We are always accepting new patients.

What Information do I need to schedule an appointment?

Your name, date of birth, address, phone number, medical history and insurance card.

What if I don’t have insurance?

You don’t need health insurance to be seen.

How much will I pay without insurance?

Ask about our sliding fee when you schedule your appointment.

Should I wear a mask?

For the protection of yourself and others, everyone is required to wear a mask at all times while in our facilities.

What if I don’t have a mask?

We are happy to provide you a mask!

What if my doctor told me not to wear a mask?

Children under two or individuals with a disability, medical or mental health condition that prevents them from wearing a mask are exempt.

Can I bring someone with me to my appointment?

If necessary, one parent or caregiver may accompany a patient.

How do I check-in?

Please remain in your vehicle and call the number displayed in the patient parking lot to alert staff you’ve arrived.

Will you take my temperature?

Yes, upon entry, with a touchless thermometer.

Video Visits

How do I check in to a Video Visit?

You may complete ECheck-in up to 30 minutes before your appointment time. Otherwise, our staff will call 15 minutes before your appointment to assist you.

Is there a co-pay for video visits?

Most likely. Your copay is set by your insurance provider or our sliding fee scale.

“Curbside” Visits

How do I check-in to a curbside visit?

Park in a “MyChart Virtual Visit” spot and call your clinic’s phone number to alert staff you’ve arrived. Someone will be with you shortly.
