Hertford Health Access (HHA) is a new health program designed to help low-income, uninsured residents of Hertford and Bertie County receive medical care. Participating physicians in our network pledge and donate medical care to patients enrolled in HHA for free of charge. HHA enrollees are linked with a primary care provider and have access to specialty care, medication assistance, lab and diagnostic services, as well as, case management and health education.
Board
The Hertford Health Access program is governed by the Hertford Health Maintenance Alliance (HHMA), a local Collaborative Network that addresses health outcomes for low-income uninsured and underinsured residents in Hertford County. Members of the HHMA board include: Chowan University, Choanoke Area Development Association, Trillium Health Resources, Hertford County Cooperative Extension, Hertford County Department of Social Services, Hertford County Public Health Authority, Hertford County Office of Aging, Hertford County Public Schools, Pregnancy Care Center of Ahoskie, Roanoke Chowan Community College, Roanoke Chowan Community Health Center, and Vidant Roanoke-Chowan Hospital.
Eligibility
The HHA program enrolls patients of the following criteria:
- Between the ages of 18-64 years old
- At or below of the 200% Federal Poverty Level
- Hertford County resident who is a patient of RCCHC
- Diagnosed with a Chronic Disease
- Linked with a primary care provider
The Hertford Health Access will utilize a systematic eligibility screening system that assures that each enrollee is a Hertford County resident who is a patient of RCCHC and meets financial eligibility criteria. Through this program, uninsured individuals will have access to Patient Navigators who will enroll eligible patients into HHA, provide them an identification card, assign eligible persons to a medical home, assist patients with an understanding of the patient responsibilities and guidelines, inform patients of the medication assistance program, and make appropriate referrals to primary care physicians and specialists in the community. Financial screenings will be conducted annually to determine ongoing eligibility.
